The Policyholder is obliged to notify the Insurer in writing about the insured event no later than three months after its occurrence. In the case of a diagnosis of a serious illness in the Insured Person, the Insured must submit the following documents to PJSC GRAWE UKRAINE Life Insurance:

• a written application for insurance payment;

• copies of the passport and identification code of the Insured Person;

• the original insurance policy;

• copies of all medical documents that would reflect the course of treatment (medical reports, medical histories, discharges, etc.);

•other documents related to the insured event (at the request of the Insurer).
